What you can expect
The following are instructions for your first visit with us:
- Please try to come 10 minutes prior to your scheduled appointment. There is paperwork that must be completed.
- Please bring all of your insurance cards with you (Primary and Secondary). We must make a copy of them for billing purposes.
- We would appreciate the completion of the history form and registration form prior to your visit. Please bring them with you.
- Bring all of your medicines with you.
- We recommend that you call your insurance company prior to your visit to be sure of your coverage. We also advise you to check with them and verify that we are providers on your plan. The providers do change rather frequently, so it is best not to trust your provider directory.
- Be prepared to pay your co-pay if you are required by your insurance company to do so.
- We do our best to avoid lengthy waiting periods once you are ready for your appointment. Emergencies, however, are out of our control and they do occur. Our doctors must respond to emergencies as a priority and we hope that our patients can have an understanding of this circumstance. Our staff will attempt to call you as soon as they are told that your doctor may be late or even unable to see you that day. We make every attempt to reach you; however, it may be possible that you have already left for your appointment.
What to Bring
- Medications (any current prescription drug you are taking - A list of exactly how you take these medications can also be helpful.)
- Medical Records (copies of records from referring physicians and test results- within last 12mo.)
- Insurance Information (including your current insurance card)
